Step One—Fill Out the Online Questionnaire

This initial questionnaire is a confidential assessment used to help Dr. Hudak understand your health history and symptoms. With the answers you provide, we can help identify your most severe symptoms and create your personalized plan.

Step Two—Determine Your Hormone Levels with a Purchased Saliva Test

Home saliva hormone tests can be used to determine your estrogen, progesterone, testosterone, DHEA and cortisol levels.

To purchase your saliva hormone test online, click here. Otherwise you are welcome to purchase one directly from us in the office. Saliva hormone tests have many advantages over blood draws, including:

• Tests can be collected at precise times
• Non-invasive and safe
• Hormones measured in saliva represent what is immediately in the body’s cells

Step Three—Schedule Your Consultation

Once the results of your saliva tests are in, you can schedule your hormone replacement consultation. During your in-office consultation, Dr. Hudak will review your questionnaire as well as your hormone levels. During this time, it is important to come prepared with any questions you may have about BHRT. Each patient’s program is created to address their individual needs and to help you maintain a healthy lifestyle.

The cost for a male and female Bio-Identical Hormone Replacement Therapy consultation is $325.00, which includes your initial in-office consultation, as well as two additional follow-up consultations by e-mail.

Step Four—The Prescription

You must have a prescription from us for BHRT. Prices are around $15-30 per prescription.

Insurance--We prefer for our patients to pay up front for services. We are happy to provide the paper work to your Health Savings Plan for reimbursement for your test and consultation.
